As you consider registering your child(ren) at MAI Leadership Academy, you can expect:

• Child’s Physical Form
• Immunization Records
• Registration Fee (non-refundable)
• Uniform Agreement Form
• Medication Form
• Food Program Application Form
• Allergies, dietary restrictions (if applicable)
A registration fee is due prior to the scholar’s start date, which is determined between the school and the parent. This is a one-time, non-refundable fee. If a scholar is withdrawn, then re-enrolls at a later date, a second enrollment fee will be required.

Uniform Policy
All MAI scholars two years old and older must adhere to the school uniform policy and wear FULL uniforms to school daily.
A FULL school uniform consists of:
1) the designated solid, colored polo shirt for that age group or grade level
2) khaki bottom
3) solid, black closed-toe shoes
If scholar is out of uniform upon arrival to school, without approved permission by school Director, parents will be notified immediately for pick up or proper change of clothing for uniform.
